CARE MANAGEMENT

  • Analyze local markets

  • Establish and maintain relationships with discharge planners and key hospital personnel.

  • Provide education to senior groups in the community.​

  • Participate alongside facilities in community-wide events.

  • Sponsor facility events.

  • Provide marketing for facilities to utilize in gaining future residents.

  • Participate in marketing planning.

  • Participate in PDPM team meetings for selection of potential SNF admits.

  • Assist MDS nurses with documentation reviews and proper coding of MDS.

  • Administer BIMS and complete sections K & GG of MDS.

  • Work alongside nursing to reduce QM triggers thereby improving facility star rating and CMI.

  • Develop programs to assist with identifying patients at higher risks for falls, contractures, pressure ulcers, and weight loss.

  • Perform weekly functional rounds with nursing to ensure timely identification of patients requiring therapy intervention.
